Requirements
What we're looking for
- Bachelor's degree in Social Work;
- Registration with the Ordem dos Assistentes Sociais (Social Workers Association);
- Experience in a hospital setting and/or training in the healthcare field;
- We value training or experience in the field of palliative care;
- Good organizational and planning skills;
- Ease of communication and interpersonal relationships;
- Empathy and sensitivity in supporting patients and families;
- Team spirit and multidisciplinary collaboration;
- Good knowledge of English.

The role
At Hospital da Luz Lisboa, we believe in person-centered care, where the human dimension is as important as clinical excellence. We are looking for a Social Worker to join our team, contributing to the monitoring and support of patients and their families throughout the care journey.
What you'll do
- Participate in the preparation and monitoring of the individual care plan;
- Contribute to hospital discharge planning and to the continuity of care in the community;
- Ensure the psychosocial monitoring and support of patients and family members;
- Promote coordination with entities and institutions within the social support network;
- Support and guide informal caregivers;
- Develop social advocacy interventions and the defense of patients' rights;
- Collaborate actively with the multidisciplinary team, participating in meetings and care projects;
- Contribute to training and development initiatives directed at the team and the institution.
